How to Apply for North Dakota Medicaid
With the rising cost of medical services and prescription drugs, many people in America struggle to maintain health care coverage. Complete an application for health care coverage. The application may be obtained from your local county social services office or downloaded from the North Dakota Department of Human Services website.

Make a copy of your photo ID, proof of income and expenses such as medical and utility bills, as well as all other required documents such as tax returns and birth certificates to submit with your application. See page two of the application for a checklist of required documents (see Resources).

Submit your completed application and supporting documents to the North Dakota Department of Human Services. The application can be mailed or returned in person to your local county social services office or faxed to 701-328-2085.
