How to Apply for a Medicaid Transportation Provider in Florida

Medicaid is a federally and state-funded, state-operated health insurance program for low-income pregnant women, children, elderly and disabled individuals. Medicaid may pay for medical transportation to and from medical appointments, as long as the transportation provider is approved to collect Medicaid payments. Medicaid will pay for emergency medical transportation, so if you have a medical emergency, do not follow these steps. Call "911" and access medical treatment as soon as possible.

  Determine whether medical transportation is covered under your specific Medicaid policy by contacting the Florida Agency for Health Care Administration at 888-419-3456.

  Contact the Commission for the Transportation Disadvantaged at [phone], Ext. 5700, or consult CTD's listing of transportation coordinators (see Resources) to obtain a list of medical transportation providers in your community. Make sure to inform the office of any special needs you have, including needing a wheelchair lift.

  Enroll in services with the medical transportation company and provide any necessary information, including your Medicaid number, so that the provider can verify your eligibility.

  Contact the medical transportation company at least 72 hours in advance to schedule an appointment. You must be ready to leave 60 minutes before your scheduled appointment or earlier if a longer transportation time is expected. If you are not ready at this time, the transportation provider is not obligated to wait more than 10 minutes at your pickup location.
