How to Apply for Medicaid Due to Medical Disability
If you have been injured on or off the job, you may be eligible to receive help from the U.S. government in the form of Medicaid. Even if you did not qualify for medical disability from your job, you still may qualify to receive Medicaid depending on your financial status and the extent of your injuries. Visit the website of the Medicaid office for your state (see Resources). Medicaid is different in each state, so you will need to fill out an application that is specific for the state where you live. If you prefer, you can visit your local Medicaid office to obtain an application.

Fill out your Medicaid application. You will need information such as the social security number and date of birth for you and every other member of your household. You will need to list any household income, including your spouse's income.

Pay special attention to the section that asks if you have a medical disability. Double check the application to make sure that all information is correct.

Fax or mail in your application to the Medicaid office listed at the bottom of your application. Or you can deliver the application by hand to your local Medicaid office.

Gather documents you will need during your interview with a Medicaid representative. These include: all financial documents such as last year's tax returns, bank statement and check stubs. You may also need a phone or electricity bill to prove that you live at your address. Obtain any medical records that pertain to your disability. You will also need your driver's license and, depending on your state, a copy of your birth certificate.

Complete an interview with a Medicaid representative. You may be able to do the interview by phone. You will be asked a series of questions about your finances, your medical disability and the amount of money that you spend each month on food and bills.
After your interview, your representative may ask you to submit financial and medical documents. Once these documents are reviewed, you will receive a letter in the mail stating whether or not you are eligible for medicaid. If you are eligible, you will also receive a medicaid card.
