How to Replace a Medicaid Card

Call the toll-free number for your state's Medicaid office. If you do not know the telephone number, you may locate it by going to your state's Department of Health website.

Select the option to speak with a live representative.

Inform the representative that you would need to replace your Medicaid card once connected. You will be required to confirm your identity by providing your name, Social Security number and date of birth.

Confirm receipt of the replaced Medicaid card. Replacement cards typically arrive in seven to 10 business days from the date of request. If the card is not received within that time period, call the state's Medicaid office to report that the card has not arrived.
