Report Medicaid fraud in New York to the New York State Office of the Medicaid Inspector General. In New York, Medicaid fraud is divided into two categories: provider fraud and recipient fraud. Provider fraud includes physicians and suppliers who overcharge Medicaid or engage in fraudulent behavior. Recipient fraud includes Medicaid recipients who engage in fraudulent behavior such as receiving kickbacks to allow providers to charge their account for services never rendered. You can submit your complaint anonymously, but the office prefers that you identify yourself so they can contact you for more details.

      Report Medicaid fraud in New York online (see Resources). Enter your name and contact information into the corresponding fields, the provider ID and recipient CIN, if available, and describe the fraud. The provider ID and recipient CIN are numbers issued to the provider or recipient by Medicaid. You can only look up a provider's ID (see Resources); you will have to ask a recipient or have access to a recipient's medical summary to get his Medicaid number. Select the "Provider Complaint" or the "Client Complaint" tab as appropriate and enter the name(s) of the person(s) committing the fraud and their contact information. Press "Submit."

      Report Medicaid fraud in New York state by phone. The Medicaid fraud hotline is: 877-873-7283. A representative will ask you for your name and contact information, the names and contact information for the persons committing the fraud and for details concerning the fraudulent activity.

      Type a statement that details the parties involved in the fraud including their names and contact information, the provider ID or patient Medicaid number of the parties involved, if available, your name and address, a summary of the fraudulent activities, the date the fraud occurred, and the names and contact information of any witnesses. Submit the statement by email, fax or postal mail.
