Colorado Management of the Medicaid Information System
Since 1974, Colorado Foundation for Medical Care (CFMC) is a review organization for the Colorado Medicaid informational system. Colorado Medicaid is public health insurance for low-income children, families, pregnant women, elderly and persons with disabilities.-
Management
-
Management for Medicaid information system is handled by three care organizations. The three organizations are Denver Health Medicaid Choice Plan, Rocky Mountain Health Plan and Colorado Access Health Plan. Each organization is separated into counties. Colorado manages two types of Medicaid systems. Basic Medicaid involves no cost for children and pregnant women. CHP+ is a low-cost Medicaid, which involves a small fee of $35 each year as of December 2010 and co-payments ranging between $1 and $5.
Benefits
-
Colorado's Medicaid system includes low or no out-of-pocket expenses for health care. As of May 2010, due to increased fraud, Senate Bill 10-167 was signed to increase efficiencies in the Colorado health care informational system. This bill also reduces the general fund expenditures for public medical benefits.

This form can be printed from online and filled out, or obtained at one of Colorado's County Department of Human/Social Services in the county the applicant resides.
