California HCFA 1500 Requirements for Ambulance Transport

Ambulances provide emergency transportation services to those requiring medical attention. Form HCFA 1500 is filed in order for patients to receive health insurance payments, such as Medicare. Ambulance workers must provide accurate information on HCFA 1500 forms. Recently, the Health Insurance Portability and Accountability Act has issued the National Provider Identifier, a number code to identify health insurance providers. California began replacing the outdated HCFA 1500 form with the CMS 1500 form that allows for the National Provider Identifier codes.

    • Ambulance workers report information on the California HCFA/CMS 1500 form detailing reasons an individual requires transport. Such information includes details about level of service, dispatch instructions if supplied and the reasons other transportation was not used instead.

    Time Frame and Patient Status

    • Information required on the HCFA/CMS 1500 form includes the physical condition of the individual being transported. The month, day, year and destination are also recorded on the form.

    Destination Transport

    • Information regarding the patient's medical history, including the date a condition first began, is required on the HCFA/CMS 1500 form. Ambulance workers must record all medical services given to the individuals, including supplies and drugs.
