How Do I Apply for an Arkansas Medicare Provider Transaction Access Number?

An Arkansas Medicare Provider Transaction Account Number (PTAN) is also referred to as a Provider Identification Number (PIN). This ten-digit number is used by the state of Arkansas to individually identify each health care provider in the state and, in turn, health care providers use this number to bill Arkansas Medicare for services rendered. The National Plan and Provider Enumeration System gathers and maintains identifying information on health care providers and assigns PTANs.       Go to https://nppes.cms.hhs.gov/NPPES/StaticForward.do?forward=static.npistart and click "Apply Online for an NPI." This form can be completed in approximately 20 minutes.

      Read the terms of the website on the NPI Application Instructions page and agree to it by clicking the "Begin Application Form" button. The "Application Security Check" form appears.

      Answer two security questions on the "Application Security Check" form. Click "Next" to go to the NPI Application form.

      Create a user ID and password on the NPI Application form. After entering your password twice, answer five security questions of your choosing from a drop-down list and click "Next" to go to the second page of the NPI Application form.

      Self-describe as either an individual or an organization by selecting the appropriate radio button and then click "Next" to go to the third page of the application, the "Provider Profile" page.

      Fill in your name, date of birth, state of birth, country of birth, credentials, Social Security number and other requested personal identifying information on the "Provider Profile" page. Click "Next" to go to the mailing address form.
