How to Locate a UPIN Number

Until 2007, Medicare identified every physician in the United States who would bill to Medicare with the the Unique Physician Identification Number. The National Provider Identifier number eplaced UPIN in 2007. However, physicians, doctors, and others can look up UPINs using tools that are still available, including both online and paper resources.

      Use ECare Online to look up a UPIN. The search is free. Enter the physician's last name, first name, State, Zip Code, and Speciality. Click the "Search" button and the search will process.

      Use HME Data's search form to look up the UPIN you need. Enter the physician's first name, last name, and state and click the "Search UPIN" button. Look at the list produced for the physician in question and his or her UPIN.

      Buy the US Department of Health and Human Services UPIN Directory. Visit the link in the Resources Section and download the order form and general ordering instructions. Follow the instructions in the downloads forms by completing the PUF request sheet, the Shipping sheet and paying through pay.gov.
