Access forms through the U.S. government's site for Medicare (see Resources). Forms should be downloaded and printed out as you will need to sign them and mail them in for valid filing.

Check on the time line if you're filing for a Medicare claim. A claim, or coverage of a procedure through the insurance, must be filed within a specific amount of time after the doctor's visit, operation or procedure.

Fill out each Medicare form or claim form with the information it requests. This will include, at minimum, the patient's name, address, social security number, medical history and any current treatments. The questions will also request the provider's name, address and certifications. Other details will depend on the unique procedure being performed at the time. Keep your writing clear and neat, and use a black or blue pen.

Keep signatures clear and legible. The practitioner's signature needs to be present on both the medical records and the Medicare form if you're filing a claim. This confirms that the practitioner who provided the service is the one filing the claim.

Confirm that the person who requested and received any medical treatment is the person whose name appears on the medical records. Consistency is important when registering for Medicare or filing a claim.

Provide copies of medical history, identification and any medical files required by the Medicare forms. This may include records of any procedures or treatments done if a claim is being filed.
