How to Correct Medicare Part B Claims

Medicare Part B is a health insurance program administered by the federal government for covering outpatient medical expenses. Elderly (age 65 and over) and certain disabled individuals are eligible for Medicare Part B coverage. If a claim is submitted with incorrect information, it is likely to be rejected and returned. A process for correcting Part B claims is available for providers and individuals who need to correct a previously submitted claim.

      Contact (800) MEDICARE if you catch an error in a claim that was submitted and not yet processed. Request that the claim be disregarded. If the claim was already processed, request that the claim be voided. If payment is received from Medicare for the claim, promptly return it. If you have already submitted a claim and it has been returned, proceed to step 2. Otherwise, skip to step 3.

      Review the reasons for the claim being returned. A claim may be returned if it contains invalid information, including listing the wrong name or other incorrect identifying information for the beneficiary, invalid service codes, invalid date of service or other missing or incorrect information. A returned claim contains a list of specific reasons why the claim was returned. Each of the reasons must be corrected prior to resubmitting the claim.

      Resubmit the claim with the corrected information.
