Go to your local Social Security Administration (SSA) office. If you do not know where your local SSA office is, go to the following website: http://www.socialsecurity.gov/locator. This website finds your local SSA office based on your zip code.

Provide your Social Security and identification cards to an SSA employee at the office and request stamped verification of your SSI benefits. The printout must be stamped and signed by SSA, since many places (i.e. state Medicaid office) will not accept it as valid proof of SSI benefits without the stamp and signature.

Check the documentation for any inaccuracies. Make sure that your name, Social Security number, date of birth and the dollar amount for SSI benefits are correct. If any of the information is inaccurate, discuss this with the SSA employee.
