Patient: Elderly woman with history of falls.

Diagnosis: Risk for injury

Goal: The patient will remain free from injury.

Intervention:

* Assess the patient's risk factors for injury, such as history of falls, difficulty with ambulation, and decreased visual acuity.

* Educate the patient about safety precautions, such as using a walker, wearing non-slip shoes, and avoiding wet surfaces.

* Remove environmental hazards from the patient's surroundings, such as loose rugs, clutter, and electrical cords.

* Supervise the patient during activities that are more likely to result in an injury, such as bathing and getting out of bed.

* Provide the patient with a call bell so they can summon help if they need it.

* Encourage the patient to participate in regular exercise to improve their strength and balance.

Expected Outcome:

The patient will remain free from injury.

Evaluation:

The patient will be evaluated on an ongoing basis for evidence of injury. If the patient experiences an injury, the care plan will be revised.
