This office calls for applications from the candidates registered with this council who have lost their permanent original certificate (issued) by this council.

Procedure:

Step1: Application:

1) Application to be made on plain paper addressed to the Registrar, Karnataka state registered Nurses & Midwives Council, No 12, Infantry Road, Bengaluru-1.

2) The applicant should send a duly filled application as shown below.

3) The application should accompany the following documents:

* Attested Xerox copy of the First page of the Bank passbook.

* Xerox copy of the first two pages of the candidate’s Aadhaar Card.

* If the candidate has changed his/her name after marriage, the Xerox copy of the marriage certificate should be attached compulsorily.

* Xerox copy of the Police complaint in the case of the lost original certificate.

* Demand draft of Rs.1000/- drawn in favor of the Karnataka state registered Nurses & Midwives Council payable at Bangalore.

* A self-addressed 24x 20cm size envelope with postage stamp worth Rs. 40/-.

4) The application, along with the above required documents, should reach this office within 15 days from the date of issue of the notification. After receiving the application, this office will verify the records and if the candidates qualify, they will be issued a duplicate certificate. After verifying all the documents the candidate will be called to the council office for completing the formality. No TA/DA will be paid for appearing in the council office.

Note:

1. If after verifying all the above documents, if any candidate is found to have produced wrong or false records to mislead the council, necessary action will be initiated as per the rules and regulations or act/bye-laws of this council.

2. Any kind of canvassing or influence will be treated seriously.

3. The certificate issued by this council is strictly confidential and it is not meant for any kind of commercial use by any person.
