Examples of Case Management Service Plans

The purpose of a case management service plan is to lay out the goals and objectives the patient/client would like to achieve during treatment. Case management service plans are used by nurses and doctors, as well as psychiatrists and lawyers. Case management service plans allow both the patient/client and medical professional to view the progress and any problems that arise during treatment.
  1. Example 1

    • Problem: Client has been suffering from binge eating.

      Goal: Client will work to view food as a source of energy.

      Objectives: 1a. Client will write down all food consumed immediately after meals and snacking.

      1b. Client will rate level of hunger at them time of eating on a scale of 1 (not hungry) to 10 (famished).

      1c. Client will keep a diary of emotions and events that may have triggered a binge episode.

    Example 2

    • Problem: Recent death in patient's family.

      Goal: Continue the grieving process while still functioning with daily activities.

      Objectives: 2a. Client will continue with open communication with family, friends, and religious/spiritual advisers.

      2b. Client will engage in more social interaction that allows him to cope with the loss.

      2c. Client will seek immediate professional help should he find himself unable to function on his own.

    Example 3

    • Problem: Dysfunctional sleeping patterns.

      Goal: Client is to work to ensure that she makes sleep a priority in order to get a sufficient amount of rest.

      Objectives: 3a. Client should engage in relaxation techniques right before bed including a warm bath, light reading, or yoga.

      3b. Client should set a time limit on telephone calls or turn cell phone off to make sure there is no interruption during the night. Family and friends should be notified of time limit.

      3c. Client should keep a journal of thoughts before going to sleep, as well as any dreams, that may interfere with quality and length of sleep.
