How do you prepare for your own person centred review?

Before the Review

* Familiarize yourself with the person-centred review process. Learn about the goals and objectives of the review, and what will be discussed during the meeting.

* Gather relevant information. Bring any documents or information that may be helpful during the review, such as your latest care plan, medical records, or notes from previous reviews.

* Think about your goals and needs. What do you hope to achieve from the review? What are your concerns or priorities?

* Prepare questions to ask. If there is anything you are unclear about or would like more information on, write down your questions so you can ask them during the review.

* Bring a support person. If you feel more comfortable, you can bring a family member, friend, or advocate to support you during the review.

During the Review

* Be open and honest. The review is an opportunity for you and your healthcare team to discuss your care and needs openly. Be honest about your thoughts and feelings, and be willing to share your concerns.

* Ask questions. If you do not understand something or if you have any concerns, do not hesitate to ask questions.

* Participate in the discussion. The review is your chance to have your voice heard. Be an active participant in the discussion, and share your thoughts and ideas.

* Take notes. If you want to remember something that is said during the review, take notes. You can also ask for a copy of the review summary or care plan.

After the Review

* Follow up on any action items. If there are any action items or recommendations from the review, be sure to follow up on them.

* Monitor your care. Pay attention to your health and well-being, and let your healthcare team know if you experience any changes or concerns.

* Be prepared for the next review. The person-centred review process is ongoing. Be prepared for the next review by keeping track of your progress and any changes in your care needs.
