You can obtain a disability identification (ID) from a state agency or a municipal mass transit system. The procedure is very similar in either case. You must locate and fill out a form, and then have a medical professional or social worker certify that you are disabled.       Obtain the necessary form from the authority that issues the disability ID that you need. These authorities include state motor vehicle departments as well as municipal transit authorities.
