How to Replace Original Placard
For people with physical disabilities, businesses and agencies reserve parking spaces to allow easy access. If qualified, the disabled person will be issued a placard to be hung on the rear-view mirror. Replacement is necessary if the placard has become mutilated or lost. Failure to display the placard while parked in a handicapped designated parking space can lead to hefty fines. Navigate to your state's motor vehicles department website and do a search for the required forms needed to replace a placard. The requirements may vary with each state, but usually you will need to complete one or two forms.

Print and complete the forms needed. In some states, the forms require personal information such as name, address, social security number and driver's license information; others require more information and may require a physician's signature.

Submit all forms and any required fees to the state's motor vehicles department either in person or by mail. Check with the local department office before going there, since some states will only accept replacement forms by mail.
