How to Get a Copy of a West Virginia Birth Certificate

The West Virginia Health Statistics Center website has an online form that you may download and print in order to access a certified copy of a West Virginia birth certificate. If you have lost or misplaced a birth certificate, you can request a new copy for a $12 fee (as of June 2010). The processing time for a birth certificate varies. If you order a birth certificate through the mail, it can take anywhere from five days to around three weeks to receive the birth certificate.

      Download the Birth Certificate Application Form from the official West Virginia Health Statistics Center website, linked in the Resources section below. You may download either the PDF or HTML version of the form. If you download the PDF version, you'll need Adobe Acrobat Reader installed on your computer to view the file.

      Print the form. If you do not have a computer in your home, visit your local library. Your library will allow you to print out the form for a small fee.

      Fill out the form with truthful information.

      Prepare a check or money order for $12. Make the check payable to "Vital Registration."
