How to Find My Connecticut CNA License Number

So you're a CNA in Connecticut and you've lost your CNA license. This can be a problem especially if you are looking for a job and need it to prove you do indeed have the education and training to be a CNA in your state. The license also proves you went through an accredited CNA licensing program in your state. Fortunately for you, it is very easy to find your CNA license number.

Instructions

    • 1

Go to www.registry.prometric.com/registry/publicCT. This is the website for the Connecticut State Nurse Aide Registry.

    • 2

Type into the box provided your Social Security number or your name and then select "Enter." Ideally, the form should pull up your information including your CNA license number.

    • 3

Write down your CNA license number on a piece of paper once the website pulls it up and put this paper in your wallet. This way you will always have it with you.

    • 4

Call the Connecticut Nurse Aide Registry at (800) 566-8668 and give them your name, Social Security number, CNA license number and any other information they ask for to verify you are who you say you are. Then request that a copy of your license be mailed to you.

    • 5

Make a photocopy of your CNA license when you receive it and put this copy in your wallet, then file the one that was mailed to you in a safe place so you don't lose it again.
