  Meet at least one of the Wisconsin standards that indicates you are handicapped. You must have yourself diagnosed by a doctor as meeting these standards. The standards are listed in the Tips section.

  Download the application form from the proper website. If your handicap is temporary, download the form for "Temporary Disabilities" from the Wisconsin Department of Transportation. Download the "Permanent" form if your handicap is permanent.

  Print the form and fill it out. Include your name, address, car license number, driver's license number, medical history and doctor's diagnosis of you as handicapped.

  Take your form to the doctor that diagnosed you and have him sign the "Eligibility" section to indicate that he believes you are eligible for a parking permit. Copy this form and keep it in your car at all times.

  Write a check for the fee for applying for a temporary permit. Each temporary permit costs $6. There are no fees for permanent stickers. Mail the form to the address on the form.
