Instructions

Contact the New York State Office of the Professions (OP), a state agency that investigates and prosecutes consumer complaints about individuals in various professions. Call the Office of the Profession's complaint hotline at 800-442-8106 and ask to file a complaint against a pharmacy in New York. Email the OP at [email protected] if you would rather file online and request a complaint form along with any additional information you may need.

Complete the complaint form once you receive it in the mail from the OP. Double-check the form for accuracy and completeness. Make sure you give as much information as you can on the form to give a complete picture of the event exactly as it occurred. Mail the complaint form to the OP office nearest to the pharmacy where you had the issue. Fax the completed form to the main OP office at 212-951-6446 if you would like to expedite the process. Make sure that you have signed the form in all appropriate places or the OP will consider it incomplete and invalidate it upon receipt.

Give the OP at least 2 weeks before you begin your own personal follow-up investigation. Contact the OP if you have not received any correspondence after you file your complaint and ask if the agency has successfully received and processed your complaint form. Ask what steps the agency has taken to correct the problem in response to your complaint. Visit the New York pharmacy again to ensure that store administrators have corrected the problem as you requested in your complaint.
