Begin the search by entering your address into the provided box. The results will provide you with an address only, or an address and map if you select the option. Each result will also provide a phone number and if available, an Internet address.

Download a health center app to your smartphone to provide you with health center addresses when traveling. If you have an iPhone, download an app from iTunes, provided by the Health Resources and Services Administration, to use your phone's GPS capabilities to help you locate a nearby center. Droid users should download the WebMD app to gain information on local health listings, maps and directions.
