There might be times when you need a copy of your medical history or other medical information that requires the assistance of your former doctor. Maybe you have relocated or changed health insurance policies and need to consult with your former doctor or have his office forward your files to your new doctor. Contact your present or former health insurance company to see if your former doctor is still affiliated with the company. Call his last known office number to see whether any former employees or staff members remember your former doctor and have any forwarding contact information.

Call the last hospital that your physician worked at to see if our doctor is still on staff or a visiting doctor. If not, ask if there is any forwarding information.

If he had a small practice as a sole proprietor, try mailing a letter to the last known address to see if the postal service forwards the letter.
