Prevention Programs

The group receiving the service is the only entity required to disclose its identity. Some programs may also ask for the date of the service, along with its type and the name of the staff member conducting the service.
Inpatient and Outpatient Programs

When a person is admitted to an inpatient or outpatient program for treatment of substance abuse, the state laws of Michigan require a name, address and phone number of the individual, as well as the patient's date of birth and gender.
