How to Obtain a Birth Certificate in Manhattan

As with all states. the state of New York has a Department of Health with an Office of Vital Records that takes requests for certified copies of birth certificates. The city of New York, however, has its own Department of Health and Office of Vital Records. If you were born in Manhattan, you will be required to submit requests for birth certificates to New York City's Vital Records Office. Kings, Queens, the Bronx and Staten Island natives are also required to submit requests to this office.

      Download the "Birth Certificate Application" from the New York City Department of Health website. The application is in PDF format, so you will need Adobe Acrobat Reader installed on your computer to view the file.

      Print the "Birth Certificate Application" form.

      Complete the form with truthful and accurate information.

      Make a photocopy of the required identification. You may use a state issued identification card, driver's license or employment ID with pay stub. If one of these forms of identification are not available, you may alternatively provide two of the following items: utility bills, telephone bill or a letter from a government agency.

      Prepare a check or money order for $15. Make it payable to the Office of Vital Records.
