How to Order a Copy of a Virginia Birth Certificate

If you are in need of a certified copy of your birth certificate, you may contact the Office of Vital Records to receive one. The Virginia Department of Health website has information on obtaining a birth certificate copy. You will need write a brief letter with information on the birth certificate you are requesting. You will also need to provide identification and a check or money order for the $12 processing fee.

      Prepare a written request for a birth certificate. You will need to include the following information: birth name, date of birth, place of birth, mother's maiden name, father's name, relationship to person on the certificate, reason for request and daytime phone number. After you have written the required information on the letter, add your signature to the letter. This is required to make the request valid.

      Make a copy of accepted forms of identification. The identification you provide may be a state-issued driver's license or identification card. If you do not have access to either of these, you may send an alternative form of identification. The Virginia Department of Health website has a list of acceptable identification.

      Prepare a check or money order for $12, payable to the State Health Department. This is the required fee for obtaining a birth certificate copy.
