How to Report an Address Change to the Health and Human Services Commission of Texas
The Health and Human Services Commission (HHSC) of Texas is the umbrella organization that oversees the five agencies of the Texas health and human services system. It is the responsibility of the HHSC to manage the majority of health care within the state of Texas. Because of this huge responsibility, the HHSC needs to maintain accurate records and it is up to you as a Texas resident to make sure the HHSC has up-to-date information for you and your family.Instructions

Speak to your HHSC adviser. If you do not have an adviser or if you are not enrolled in HHSC at all, call 2-1-1 free on your Texas phone or toll-free at 877-541-7905. In each case, you will be prompted to provide certain information that will direct your call to the proper department.

Locate an HHSC office by using the Texas HHSC Office Locator database (yourtexasbenefits.com). You can report a change of address to your adviser in person, on the phone, with a letter, or mail an official HHSC form H1019, "Report of Change" to your local office.

Follow up your change of address by calling or visiting your adviser or closest HHSC center about a week after reporting the address change. If your address change has not been registered, let your adviser know immediately to prevent any lost mail or mistakes on your medical coverage.
