How to Get a DOT Health Card

The U.S. Department of Transportation requires applicants to present a health card when applying for a commercial driver's license. The applicant must get a physical examination by a licensed physician that is certified by the DOT to give the physical examination required for the health card. The physical exam results are good for a period of 24 months. The physician may also issue a medical certificate that is valid for less than 24 months when monitoring certain medical conditions.

      Search the Internet for a DOT-certified physician. Make a list of physicians in your area.

      Call the doctor's office to schedule the DOT physical exam. Go to the doctor's office at the appointed time.

      Submit to the physical exam. The doctor will fill out and give you the DOT health form after successful completion of the exam. This DOT form is the health "card" that is needed to obtain a commercial driver's license.
