Causes

The cause of bags and puffiness around the eyes is currently unknown. Contrary to popular opinion, they are almost certainly not caused by lack of sleep; however, they may be connected to alcohol and drug abuse, aging or may simply be the result of a genetic disposition towards puffy eyes.
Surgery

The operation to remove bags and puffiness from around the eyes is called a blepharoplasty. As with many plastic surgeries, it is not generally a necessary surgery, but it can increase a patient's feelings of self-confidence and self-worth. As plastic surgeries are not usually covered by health insurance, you may want to check with your insurance provider to see if they cover blepharoplasty.
Upper Eye Surgery

During a blepharoplasty of the upper eye, the surgeon examines the creases of the eyelid to determine where to make the incisions. By cutting at the creases of the eyelid, the surgeon ensures that the minimal scarring results. After making the incision, loose skin, excess fat and muscle are removed, and then the wound is sealed using fine sutures to minimize scarring.
Lower Eyelid Surgery

During a lower eyelid blepharoplasty, the surgeon makes incisions at the lower lash line and the creases of the lower lid. The surgeon removes excess skin, fat and muscle, and then seals the incisions with fine sutures. If the problem is puffiness rather than a bag around the eyelid, then the surgeon may make an incision on the inside of the lower eyelid instead. While both of these procedures cause minimal scarring, any resulting scars should not be noticeable.
Recovery

The area affected by the operation will swell for a few weeks afterwards. The wounds will need to be regularly cleaned for a period of a week or two, and your doctor may give you an ointment to apply to combat skin dryness. Any permanent stitches will be removed three or four days after the surgery. Any temporary stitches will dissolve on their own.
